Authorizations

If your insurance is highlighted above, you are required to get an authorization before services can be rendered. Without prior authorization you will be responsible for payment of service at the time of service.  We at Livewell understand this process may be cumbersome, however, the authorization is necessary for you to access your mental health benefits and to cover the cost of your benefits under these insurance plans. Please see below to determine how to get your authorization.

Kaiser – please contact your primary care provider for a referral to Livewell Behavioral Health, or call 1.559.448.4620 and ask for mental health triage staff.

Beacon – Please call the number on the back of your medical card and ask for an authorization for behavioral health.

Magellan – Please call the number on the back of your medical card and request an authorization for behavioral health.

Halcyon – Please call 1.888.425.4800 and request an authorization for behavioral health.

Payment

Cash, check and all major credit cards accepted for payment.

Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.

I’d recommend asking these questions to your insurance provider to help determine your benefits:

• Does my health insurance plan include mental health benefits?
• Do I have a deductible? If so, what is it and have I met it yet?
• Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
• Do I need written approval from my primary care physician in order for services to be covered?

Livewell Behavioral Health Outpatient Services is an equal opportunity provider.

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.

You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

Cancellation Policy

Livewell Behavioral Health has a no show/late cancellation policy. For cancellations of less than 24 hours notice or failure to show for a scheduled appointment, an $80.00 fee will be assessed.
